We have a number of vacancies working in our Home First team at our Assessment Hub in Perth House and working across various locations in Derby including:

Home First team
Join our Home First team and you will be working in the community, in customers own homes across Derby and in our Assessment Hub at Perth House – being responsive to the needs of our customers by helping to prevent hospital admission and facilitating hospital discharges.
You’ll be involved with providing support and showing customers how to do things differently to maximise their independence with such tasks as personal care, dressing, undressing, making meals, medication, shopping and day to day activity in our short term service.
You will also be involved in supporting our partnership work with DCHS. (Derbyshire Community Health Services) and working together with our NHS colleagues.
